Black Jack is a medical mercenary, selling his skills to the highest bidder. He is a shadowy figure, with a black cloak, eerie black-and-white hair, a scar across his face and partially black skin. Black Jack cures patients indiscriminately, from common folk to presidents and yakuza leaders. To his VIP patients, he charges absurd sums. All this has given him a reputation for callousness and greed which he gleefully cultivates. However, to the reader it is clear that Black Jack actually is a good man: he is anti-wealth and anti-prestige, and believes he is actually doing rich people a favor by removing their material wealth. The opposition to wealth and power is a common theme in Tezuka's work: powerful men are almost always portrayed in a negative light. Throughout his life, Black Jack learned some harsh lessons and has to face the reality that there are times he cannot go against the forces of nature. In difficult and tricky situations, he is a quick thinker and is able to fool others easily. Besides being an expert in surgeries, he is also a skilled combat fighter who makes use of his surgical tools that he hides underneath his cape. Although, he is perfectly capable of fighting with his bare hands, even delivering a single knock-out punch to the stomach at one point. He also shows that he is highly responsible for his patient's safety and well-being, which explains his high pricing. He even goes to the lengths of deceiving his patients to make sure they heal properly. He also cares for and is deeply appreciative of people who change his life, such as Dr. Honma, who saved him through surgery once. Black Jack is also very prideful and hates being made fun of, or things he deems important desecrated.

Aside from possessing the strength of 100,000 horsepower (later 1,000,000) and the kind personality of a child, he has what are called his "seven amazing powers": jet-powered flight, multilingualism, analytical skills, headlight eyes, super-sensitive hearing, hidden weapons in his back and the ability to tell if a person's good or evil. In addition he has a built-in geiger counter and shortwave communication system.

The young Prince of Silverland, Sapphire is in reality a girl, but must pretend to be a boy in order to inherit the throne and prevent the evil Duke Duralumon from taking power. Her personality is more refined than tomboyish, but she's good with a sword and at night fights crime as the masked Phantom Knight. She's good-hearted, has a strong sense of justice and is competent and level-headed as well. Although most of her time is spent as Prince Sapphire, later in the series she is able to go about as a girl (wearing a blonde wig) in the persona of the Princess, Prince Sapphire's sister. In this way she is able to court Prince Frank.

Dr. Ochanomizu, a recurring character in many of Osamu Tezuka's manga, including Astro Boy, can be seen "lecturing" at Narita International Airport, outside of Tokyo, on HIV and safe sex. Dr. Ochanomizu was most famously seen as the mentor to Astro Boy and the creator of Astro's sister and parents.

Melmo was given a bottle of candy from God. The blue candy turns Melmo into a lovely 19-year-old version of herself, while the red candy turns her back into a child. Combining the two turns her first into a fetus, then into an animal of her choosing. When Melmo ages from 9 to 19 her clothes don't grow with her, usually resulting either in shredded clothes, or skin-tight children's clothes on the body of an adult woman. Melmo is a kind-hearted young girl who is frequently torn between her actual age and the things she encounters and wants as her older self.
